Sciatica: pain that travels down the leg

"Sciatica" is two things at once — it's a condition, and it's a descriptor for pain that travels into the leg, usually on one side or the other. Naming the pain is easy. The useful question is why it's happening.

Why is there pain going into the leg?

It might be a bulging intervertebral disc compressing a nerve root that feeds into the sciatic nerve distribution. Or it might be compression from the piriformis muscle — the sciatic nerve pierces through the piriformis, so when that muscle clamps down, you get sciatica from what's called piriformis syndrome. (Think of sitting on a wallet in your back pocket in a vehicle for eight hours.) And in some cases it's a combination of issues that leads to this type of pain.

Which of those is driving your leg pain changes what should be done about it — which is why the first visit is an evaluation, not a one-size-fits-all protocol.

What can be done?

In our office, treatment is built from gentle techniques that address the surrounding musculoskeletal system, matched to what the evaluation actually finds:

  • Lumbar flexion on the auto-flexion table — gently tractioning the low back to ease pressure on the discs.
  • Deep muscle massage — hands-on work to relax the affected muscles, including the piriformis when it's part of the problem.
  • Specific stretches — targeted to your presentation, not a generic handout.
  • Specific adjustments to the spine — precise, gentle, and only where they're warranted.

Often it's the combination of these — along with walking and other movement-based activity — that helps sciatica settle down.
